Short version: he was a pirate who drowned and came back as a kind of water spirit but he's still definitely a pirate. Feel free to ignore what he has on (or use it, your call). I have a lot of ancients so I can appreciate minimalist, but I also down for using all the slots too. I don't have a particular opinion on how many of his eyes are visible.
