I just used Bolder Bolt with the Fae to finish off a Nightsky Furian. He was the last mob there as you can see. The Fae would have been under the effects of Meditate Exhaustion and Guard.

Edit: The UI vanished as that screen started floating away, too.

This occurs when drawing goes wrong and something doesn't get translated back. I don't know that it's something I can fix - best I can do is run some tests using the scenario you mentioned (Thanks for that) a few times and see if it occurs.
