seeing this image gave me intense memories of pure fear. i remember having to close my game and walk away upon making it into limbo. the eyes are deeply unsettling to me for some reason. i thought exploring the dimensional doors was cool but the fact that limbo was attached to that kept me from doing so
I don't know if it counts as a horror game or not, but the mere second chapter of Call of The Sea spikes up on the creepy meter rather suddenly; while the rest of the game still had some creepy vibes, "The Well" was honestly the creepiest part. But maybe that's partly because I didn't know I was supposed to look at something else in there and so the noises didn't stop like they were supposed to

In pokemon Firered, I always turned my volume off when entering Lavender Town, not uncommon I know, but the music is what spooked me most. Everything else there, the ghosts and the graves, that was par for the course. I just couldn't handle the music.

In a similar vein, Endless Ocean. I know right? It's basically just an ocean simulator, nothing scary about it. Until you Get near the giant trench and it starts playing the most ominous music imaginable. Pitch black, straight down, and having to go DOWN there for a mission, I had to work up the courage to do so. Even after that, there's the weird ominous messages you'd get every time you picked up a statue (playing the same music as near the trench), and by that point I was so spooked I never played it again.

Someone already explained the deal with Gaster, but the thought of something in a video game being aware that I, the player, exists. It does nothing but give me the heebie jeebies. I'm not sure whether Yume Nikki counts, but Uboa elicits that same feeling.

And honestly, probably a lot more than this.

Two final levels in Dog’s Life ps2, the dog pound and the factory.
The dog pound is like a hell itself, then the factoty is build to make a cat food out of dogs. Those circular saws had blood on them.

That was a pretty big jump from the rest of the game about a dog collecting bones and farting.

Here's another one. Only applies to 3D games in my case though. You know, what makes me really uncomfortarble in video games? What gives me some strange sense of dread? Giant vortices. Be it black holes, massive whirpools or similar. Shoutout to The Beginners Guide, which has a little chapter which starts in a lecturing hall, locked in facing the podium. Eventually the perspective shifts to the speaker. how could I forget about the final boss of Earthbound omg

the game's got a lot of surreal stuff in it, but Giygas definitely used to freak me out when I was a kid, especially with all the edgy theories about him at the time lol

edit: also that one bit in mgs2 where the colonel and rose turn out to be fake actually and keep calling you to spout nonsense such as straight up telling you to turn off your console. it's probably not as effective on me now but it's certainly weird, if nothing else
