Doctor Telesphorus (or, as he would prefer to be called, "Dr. Tell") follows a code of ethics in his medical practice that seems rather disconnected from his Plague origins. Namely, his bedside manner is impeccable, if not a touch overbearing, and he prioritizes comfort over pain. These standards might seem quite low for a doctor of any worth, but Telesphorus knows all too well how many of his peers fail to clear this bar. Having been sickly himself for most of his life, the good doctor has quite literally gotten a taste of his own medicine - something he thinks more physicians could stand to experience themselves. Of course, this dedication to kindness and etiquette manifests itself in his strange insistence on calling everyone "sirs" or "madams", as well as his use of quirky phrasing for even the simplest of things. But no one can argue with the quality of care that he provides, and so long as his results remain satisfactory, nobody pays it much mind.

An alternate argument is that Telesphorus disguises the weakness that his illnesses have wrought over his body with his larger-than-life personality, but it's best to not bring up this theory in polite company.
