Here's the form, with some explanations!
For each graphics you are ordering you'll want to fill out another form.
Eg. If you want a profile banner and two badges, you'll need to fill out three forms.

Type: Profile banner, signature, hatchery/thread banner, breeding pair card, etc.
Dimensions: How big do you want the image to be? Layout: My premade template, or a custom? (For custom you'll want to describe to me what you want!) This is for breeding cards. Leave blank if it does not apply. Colour(s): What colours would you like? Dragon(s): If any of your dragons are being included, especially on a breeding card. Image(s): Any images you want included, whether you've got some for me to use or if you have an idea of what I need to look for. Make sure the images are available for manipulation; Creative Commons Attribution is an example of what will be used. Shape(s): Want shape cut-outs or a shape to your graphic's border to add even more flair? Main Text: Username, title, etc. For hatchery/thread banners, be sure to mention the titles of every sub-banner you need. Don't forget to mention which font you would like! When I open a slot, I will ping the first on the list.
If there is no response or a decline within 24 hours, I will ping the next user, wait another 24 hours, and so on until the slot is filled.
