The only effect I've noticed is that it highlights certain items in the database when they show up in a search. Does it do anything else? Can I access a list of my favorites anywhere?
Edit: I have discovered the Display Type option

in both the game database and your own hoard there is a filter to show only the items that you've marked as a favorite
in the database it's under the display type option, while in your hoard it'll be the star in the top right
