The site's policy is that anybody should be able to return to their account at any time and still have their stuff, that includes username.
Names might be a bit different if the user's been banned, but otherwise... that's the rule.

Not quite sure what you mean with policy on something? A name that is taken is taken. If you want that name you're out of luck.

The way to tell if a user has been banned is if you can't post on their user page.
