Blackblood (he's getting a new name soon) is the grim reaper of my clan. He guides the dead into their chosen afterlife or into reincarnation, chases down lost souls and ghosts, and is not very fond of necromancers. However, his outfit is....lacking, and he isn't as spooky and intimidating as he should be.
So, I have one slot left for another apparel piece - what should I give him, what should I take away, or should I just leave him as he is???

I'd say try adding a reaper guise if you're going for a more 'classic reaper' look?
Play around with that a little, maybe?
I do like what you have though. It's a very unique look!

I think a red riding hood in place of the whisperer's cowl and a ravenskull broadsword in the last apparel slot would make him look much fiercer.

I am actually fond of the green hood, personally, but the black one would also fit the part. Probably. Red miiight work, but I feel like that could be overpowering.
Perhaps Skeletal Chimes would do the trick? It'd sure tie in nicely with the skulls on the Tatters, plus it would add some much needed busyness to the wings imho.
Should you acquire another apparel slot at any point, you might wanna look into Black Linen on the tail too. Honestly though, if it were my dragon, I would swap the Black Linen pieces for Bloody Bandages like you've got on his chest.
Here's hoping you can figure out something you're happy with!

There are some good ideas above, but I guess it boils down to what -sort- of psychopomp figure you're trying to aim for.
Classic reapers, you'd be looking for hood/mantle, or maybe one of the guises, and a lot of the Plague festival apparel would work well.
There's the various little cloth wrap items, of course, and the cobwebs. You might even consider some of the moonscale armor if you're wanting some things that look a little bony/spiky for his tail and wings?
Here's the dragons I could find in my lair that might help a bit with ideas for robes and semi-shamanistic things.
