Biography
Strawberry tongue - a physical exam finding in Kawasaki disease, a small-medium sized vessel vasculitis resulting in acute febrile illness. Associated with coronary artery aneurysms in children if untreated! (20-25% risk. Increased risk if male baby < 1 year old, fever > 14 days, or hyponatremic)
- unknown cause, possibly post-viral infection?
- increased risk in Asian boys, peak age 2 years old
- presentation: fever x4-5 days, with 4/5 of:
Treatment:
- IVIG
- ASA 81 until fever is controlled x2 days, then 3-5mg/kg daily until f/u ECHO (4-6 weeks later) is normal
- Dipyridamole - anti-platelet therapy for patients deemed too high risk for thrombosis

IF HAVE BABY DERGS:
Scarlet Fever, usually seen in children, which is basically an overreaction to the toxin produced by Group A Strep.
Symptoms:
- Pharyngitis, 12-48 hrs from onset of sore throat
- blanching, sandpaper rash which first appears in skin folds (neck, groin, axilla, anticubitum)
- pallor around lips and mouth
Untreated, the rash lasts 4-5 days and their skin peels from hands and feet over the course of 2 weeks.
Tx: at least 24 hours with antibiotics (penicillin family)
- unknown cause, possibly post-viral infection?
- increased risk in Asian boys, peak age 2 years old
- presentation: fever x4-5 days, with 4/5 of:
- Cervical lymphadenopathy
- Polymorphic rash (NOT vasicular)
- Conjunctival Injection
- Extremity changes (erythema, edema, induration)
- mucus membrane changes (Strawberry tongue, red and cracked lips)
Treatment:
- IVIG
- ASA 81 until fever is controlled x2 days, then 3-5mg/kg daily until f/u ECHO (4-6 weeks later) is normal
- Dipyridamole - anti-platelet therapy for patients deemed too high risk for thrombosis
