I hate sneaking and getting caught, especially if it means starting over from the beginning.
As for specific examples, the monster houses in the mystery dungeon games.
You head into a perfectly normal room, and then really intense music starts playing, an insane amount of enemies drop from the ceiling, and they proceed to slaughter you.
The WORST part? The dungeons are randomly generated. So you never know where one will pop up.
And if you're REALLY unlucky, then a monster house spawns in the room with the STAIRS to the next floor.

and especially when the stairs TAKE you to one of those rooms :P it's only happened to me once, but it's terrifying to drop into the room, notice all those Gummis, and realize what that means. A heartbeat later, enemies have surrounded you.

Oh, UGH. Not THOSE. I play Gates to Infinity. Sometimes you'd walk around, just enter a room, and on the bottom screen see a bunch of items in the room; awesome, right? Free loots of berries and seeds and orbs! ...take another step, and there are so many enemies that you're probably dead meat. It's just plain startling just to see them come out of nowhere what seemed to be a normal room with an unnatural lot of items laying ahout. Yikes.
But hey.
Don't forget the Kecleo (sp?).
I have made some fatal mistakes with his shops... Worse when it's an accident.
I decided NOT to buy one of his items after taking it, so I wouldn't buy it , and I thought he'd just naturally take it back.

the centaurs are just fantastically repulsive, arent they? i cant think of an enemy in a game more visually horrifying than centaurs.
i think the thing that scares me the most about super mutants is their permanent rage expression lol. even after you kill them theyre still making that face. ive heard that fawkes is a great companion and i like him as a character but sheesh i dont want to look at that all the time lol (this is coming from someone who loves ghouls)

Harvestable i havent got to play BotW so i'm not familiar with what youre talking about. ;_; ive been watching game grumps play it but they havent gotten to that part yet. any section in video games that is purposely meant to be disorienting is never a good time though.

Naettleleaf omg freakin monster houses. theyre so intense. a strategy i use is to put on the scarf or scope or whatever it is that lets you see where items are on the map. if you see a big cluster of items you know that thats the room to avoid.

Okay, don't get me wrong here, it's not that scary now, but Pokemon Pearl was my second video game ever (After The Legend of Starfy) And my small self was TERRIFIED of the old Chateau. Even now the music, and weird ghosts that disappear kinda freak me out. Just listen to this:

Scariest part of Pokemon still is probably when you did that old Missingno glitch thing and looked at the Hall of Fame, only to see a Pokemon somehow have level replaced with age.
Or maybe N's toyroom. That was legit the only time when the plot actually did something. D/P/Pt did a good job with replicating those weird moments though, kinda felt like the god rumors of old.

Roguelike rule 1: If it looks like a trap it most likely is.
Roguelike rule 2: Fools rush in. Look at your escape options first like it's a fire drill.
To be fair it's like half the challenge, negotiating the too hard. It can scare the crap out of me, but only because death is permanent. :P
